Transfusionassociated graft versus host disease (Ta-GVHD) is a rare but mortalityrelated transfusion reaction. Generally in the last month, blood transfusions occur with skin rashes, jaundice, diarrhea and pancytopenia. Since there is no treatment, avoiding unnecessary blood transfusion or irradiating blood products are the most important prevention methods. In order to emphasize the importance of questioning the history of transfusion in the etiology of pancytopenia, a case of Ta-GVHD was presented in a patient who presented with rashes, fever, jaundice and pancytopenia.
