A High Voltage Electrical Burn of Lung and Liver Parenchyma

High voltage electrical trauma may cause severe visceral injuries. Although rare case visceral injury associated with electrical injury is presented associated with a high mortality rate. We report a case of direct electrical injury to the lung and liver parenchyma, in a car washer who sustained highvoltage electric injury and long-term exposure (20-30 Minute) on the wet floor while working.

An 18-years-old boy was transferred from state hospital to our intensive care unit, 2 hours following the accident, with high-voltage burn injuries. The victim loss of consciousness. Initial management consisted of tracheal intubation, mechanical ventilation and sedation. On examination he had facial flash burns, a 3 cm2 contact charred wound on the right and left palm and left foot. The thoracic and abdominal walls were intact. On day 4, temperature was 39,5 ºC. Blood cultures were positive and there were progressive decrease in mean arterial pressure.
