Our Results of Microfracture Method in The Treatment of Osteochondral Lesions of Talus

Objective: Mid-term results after treatment of talar osteochondral lesions with arthroscopic microfracture.

Materials and Methods: This study includes 10 patients with osteochonral lesion of talus treated surgically with microfracture technique. Mean time between the initial complaint and surgery was 10.7 months. Previously, conservative treatment has been used in eight patients. Lesions were rigth ankle in six patients and left ankle in four patient in posteromedial side of talus. According to Berndt and Harty radiological classification, lesions were evaluated as stage II in 4, stage III in 5 and stage IV in 1 patient. Debridement and microfracture were applied in patients with stage II and stage III lesions, and curetage and microfracture were applied after fragman removed in patient with stage IV lesion. Ankle excercises were showed and weigt bearing not permitted for six weeks.

Results: Clinical results were evaluated according to Berndt and Harty classification. Clinical results were good and excellent in all of four patients with stage II lesions. For five patients with stage III lesions, results were evaluated as good and excellent in three and moderate in two patients. One patient with stage IV, bad result was established. Time between the initial complaints and surgery were short in all patients with good results.

Conclusion: If conservative treatment failed in patients with stage II and stage III osteochondral lesions of talus, early microfracture application is an efficient and successfull treatment options to cause lower morbidity rate because of minimal invazive surgery and to allow early return of daily activities.
