Pilomatrixoma, which is also called calcifying epithelioma of Malherbe, tricholemmal cyst, trichomatrixoma is a benign, cutaneous, calcifying tumor originating from pluripotential precursors of hair matrix cells. It is known to occur in children but it can be seen at any age. Pilomatrixomas are most commonly found in the head and neck area. Correct preoperative diagnosis varies from 11 to 50%, so it is a diagnostic pitfall clinically and on cytology. The risk of misdiagnosis of pilomatrixoma as a malignant tumor can cause extensive surgery. In this report a patient with a mass at her parotid region, which was thought as pilomatrixoma after a magnetic resonance imaging and fine needle aspiration cytology, was presented through researching the literature. She was treated surgically.
