Prognostic Significance of HbA1c in Diabetic Foot Ulcer Healing

Objective: In this study our aim was to investigate the prognostic value of HbA1c in wound healing and its correlation with diabetic comorbidities in patients with diabetic foot ulcer.

Materials and Methods: Records of patients evaluated in Diabetic Foot Council with the diagnosis of diabetic foot ulcer between January 2007-December 2011 who were suggested surgical debridement are retrospectively analyzed. During the wound healing process, patients who had at least two HbA1c values were included in to the study. Diabetes dependent comorbidities were recorded. Diabetic foot ulcers were graded according to Wagner-Meggit classification. Patients were divided into two groups; completely healed lesions and amputated cases.

Results: Thirty cases (25 males,5 females) who had satisfactory records and enough clinical follow-up period were included in to the study. Mean age was 55.9 (37-88). Mean follow-up period with the diagnosis of diabetes mellitus was 10.4 years (2-30). Mean clinical follow-up period was 153 days (30-320). Diabetic foot ulcers of 25 patients successfully healed with surgical debridement and medical treatment however ulcers of 5 patients were amputated at appropriate levels. Mean HbA1c levels in the completely healed lesion group was 8,29±1,23 and 11,16±2,78 in the amputation group.

Conclusion: In our study, mean HbA1c levels were higher in the amputation group, however the difference was not statistically significant due to low number of patients in the amputation group. We believe this difference deserves investigation in larger patient series.
