Cotard is a syndrome that is characterized by nihilistic and immortality delusions, depersonalization, derealisation, and suicidal thoughts. It has been described in organic brain diseases and in functional psychiatric disorders. Understanding Cotard syndrome and its connection to other neurological conditions is essential to identify appropriate treatment modalities. In this case report, we discussed Cotard delusions in a young male patient with schizophrenia and the treatment process of schizophrenia with these delusions in the light of the literature. In Cotard syndrome, electroconvulsive therapy may be beneficial by insufficiency of antipsychotic treatment.
