Case Report: Neuro-Behcet Disease Presenting with Epileptic Aphasia

Behçet\'s Disease (BD) is a multisystemic disease that manifests itself with various clinical conditions, the cause of which is unclear. Central nervous system involvement is called Neuro-Behçet Syndrome (NBS) in BD. Although epilepsy is a rarely observed condition in NBS, clinically, only aphasia is a rare condition in seizure presentation.

A 49-year-old female patient was admitted to the clinic with an intermittent speech disorder that started a few months before she was admitted. The patient has been followed up with the diagnosis of Behçet\'s Disease for four years and has been using prednisolone 5 mg 1x1 and azathioprine 50 mg 1x1. EEG was performed for the differential diagnosis of epilepsy presenting with speech disorder. In the routine EEG examination, intermittent slowing of theta frequency was observed in the left temporal area. When the patient\'s anamnesis and examinations were evaluated together, it was found to be compatible with aphasic epilepsy, which was thought to be primarily related to NBS. The patient was started on valproic acid 1000 mg/day for treatment, and the described speech disorder was not observed during the follow-up of the patient. If primary neurological involvement is observed in BD, this condition is called Neuro-Behçet Syndrome (NBS). Neurological involvement has been reported in 4-49% of cases; however, other large-scale studies have actually shown this to be between 3-9%. In a large series of NBS cases, epileptic seizures were shown to be quite rare. Although aphasia or speech disorder is a common ictal or post-ictal symptom, it is extremely rare for a seizure to present with aphasia alone.

This case is presented to raise awareness about seizures, a rare clinical condition seen in NBS, and ictal aphasia, a rare form of it.
