Evaluation of the Relationship between Right Ventricular Dysfunction Findings Obtained by Computed Tomography Pulmonary Angiog-raphy and Clinical and Radiological Severity of Pulmonary Thromboembolism |

Objective: Determining the severity of the disease and right ventricular dysfunction in the treatment of pulmonary thromboembolism (PTE) is very important in terms of the necessity of rapid reperfusion therapy. It has been shown that the right ventricle/left ventricle (RV/LV) ratio obtained by computed tomography pulmonary angiography (CTPA) can be used in the detection of right ventricular dysfunction (SVD) in PTE. This work was performed to evaluate the relationship between RV/LV, which will be measured by CTPA, and simplified pulmonary embolism severity index (sPE-SI) and pulmonary embolism obstruction index (PABTOI).
Material and Method: Demographic characteristics, risk factors, sPESI values, RV/LV ratios obtained from CTPA and PABTOIs of patients diagnosed with PTE in the chest diseases clinic between 2019-2020 were calculated retrospectively. The relationship between RV/LV and PABTOI and sPESI intensities was examined. Results: A total of 47 patients, 22 males and 25 females, with a mean age of 61.1±15.9 years were included. RV/LV<1 was found in 24 (51.1%), RV/LV was between 1-1.5 in 19 (40.4%) and RV/LV>1.5 in 4 (8.5%) patients. A statistically significant correlation and correlation was found between RV/LV and sPESI (p =0.014; p <0.001, r =0.516). In addition, when the relationship between RV/LV and PABTOI was examined, it was observed that there was a statistically significant relationship and correlation between them (p =0.002; p <0.001, r =0.584). Conclusion: It has been shown that the RV/LV ratio plays an important role in determining right ventricular dysfunction, as well as an important determinant of the clinical and radiological severity of PTE. The severity of the disease and right ventricular dysfunction can be determined in the early period by measuring the RV/LV ratio, which can be calculated simply by CTPA. |
