Can Total Ig E Predict Psychiatric Comorbidity in Patients with Chronic Spontaneous Urticaria?

Objective: Psychiatric disorders such as depression and anxiety frequently accompany patients with chronic spontaneous urticaria (CSU). In our study, we aimed to evaluate the effects of psychiatric conditions on urticaria and the relationship between clinical and laboratory parameters.

Material and Method: A total of 320 patients who were admitted with CSU in the adult allergy clinic were included. Demographic data, disease duration, severity, quality of life and biochemical results of the patients were evaluated. Psychiatric disease status was screened with Hospital anxiety depression scale and Beck depression and anxiety scales were applied to those deemed at risk.

Results: Among the patients, 72% were women and the median age was 28 years. A psychiatric comorbidity was detected in more than half of the patients (n =180, 56%). The disease duration was longer and urticaria activity score-7 was higher in the group with psychiatric comorbidity (p <0.001, p <0.001). The quality of life of these patients was more impaired and their response to antihistamine treatment was lower (p <0.001, p =0.003). Total Ig E values were found to be significantly higher in the group with psychiatric disorders (p <0.001). There was no significant correlation between the severity of psychiatric disease and total Ig E levels.

Conclusion: It is recommended that to refer patients in CSU, especially those with high total Ig E levels, to a psychiatrist for psychological assessment in terms of diseases such as depression and anxiety. Further studies are needed to prove that this multidisciplinary approach will provide more successful outcomes in the management of urticaria.
