Enteral, Parenteral and Combined Enteral – Parenteral Nutrition in Patients with Sepsis in İntensive Care Unit: Insulin Requirement and Assesment of Blood Glucose Levels

Objective: Glycemic control is essential to successful treatment for critically ill patients in intensive care unit. Exogenous insulin is often required to maintain normoglycemia after enteral and parenteral nutritional support.

Material and Method: Randomly selected twenty people enteral group, twenty people parenteral group and twenty group combined enteral – parenteral patients were included in the study; who were hospitalized in the intensive care unit for surgical and internal reasons and were diagnosed with sepsis according to the \"Surviving Sepsis Campaign\" protocol.

Results: The blood glucose levels of patients who were fed enterally on the 1st and 3rd days were more stable and the number of hyperglycemic attacks was significantly lower than that of the other two groups. Also, the insulin dose used on the 3rd day in these patients was significantly (< 0.05) lower than in the other groups.

Conclusion: The dose of exogenous insulin and the number of hyperglycemic attacks are lower in enteral nutrition grups in intensive care patients with sepsis diagnosis compared to Parenteral and combined groups. Based on these results, nutrition and the route of administration of nutrition influences the circulating on insulin and blood glucose concentrations patterns. These findings may be aid to clinicians how to deal with sepsis if needed enteral and parenteral nutritional support.
