A Rare Suicid Method, Apple Kernels: A Case Report

Cyanogenic glycosides including amygdaline are naturally occurring herbal toxins. Apple seeds contain amygdalin, a potentially toxic compound. Orally amygdalin is hydrolyzed to cyanide with beta-glucuronidase in the small intestine and leads to cyanide poisoning. Suicidal attempts which are rarely used and involve complex methods are particularly important and oral intake of toxins that can turn into cyanide can also be considered in this context. Personality disorders are frequently diagnosed in cases presenting with suicid attempt and borderline personality disorder is one of the highest risk in terms of suicid attempt in the community. In this case report, a patient who attempted suicid with apple kernels and considered borderline personality disorder is presented. It is aimed to demonstrate that the early recognition and treatment of signs and symptoms of cyanide toxicity is vital for a successful clinical outcome and to draw attention to rare suicidal methods and possible risk factors, especially in patients with personality disorders.
